DOCUMENTS TO BE ATTACHED
Four attested photocopies of:
a. Academic Certificates (SSC/F.A/F.Sc/B.A/B.Sc/B.Ed/B.Com)
b. Character Certificate from Head of Institution last attended.
c. Detailed Mark Sheet of Qualifying Examination.
d. Four recent passport size photographs.
e. CNIC (Applicant & Father’s)
f. Local/Domicile Certificate.
g. Affidavit (in original) on Judicial Paper duly countersigned by the 1st Class Magistrate as per specimen given in the Prospectus.
h. Candidate having qualified from other Board/University have to submit in addition to the above a migration certificate (in original) from the Board/University.
IMPORTANT INSTURCTIONS

• Fee once deposited cannot be refunded in any circumstances.
• Any incorrect or false statement made in this form may lead to disciplinary action or cancellation of
  admission at any time / stage.
•  Incomplete application form will not be entertained.
• Application Form should be duly filled in by the candidate himself/herself. If at a later stage it is revealed that the form is not filled by the applicant by himself/herself may lead to cancellation of admission.
•  Applicants applying for admission shall submit their applications dully filed in all respects along with all the required documents as per details given in the prospectus of University of Loralai
