REQUIRED DOCUMENTS
Matric & Intermediate Certificate. MBBS degree & detail marks sheets of all professional examinations. FCPS Part-1 passing certificate/result of above cited CPSP programmes accordingly.
PM&DC valid permanent registration Certificate. House Job certificate with exact dates from the concerned Medical Superintendent/Administrators. Distinction Certificate. Copy of publications (if any) in approved journal from PM&DC/PMC/PubMed Indexed & HEC. Domicile Certificate.
Computerized National Identity Card (CNIC). Only Relevant subject/specialty related experience certificate from Teaching Hospital recognized. One passport size photograph duly attested from back side. Those having foreign qualifications are required to submit HEC/PMDC equivalency certificate.
SZFPGMI Lahore Admission 2023 Application Forms
Application and academic info form can be downloaded from the website, www.skzmdc.edu.pk processing & application fee (Rs. 2.000) shall be submitted through Intra Bank Transection (IBT) in favour of Deputy Dean Shaikh Zayed Postgraduate Medical Institute. Lahore Account No. 3154820902 National Bank of Pakistan (NBP) Shaikh Zayed Hospital. Lahore Branch code 1707 and forms can be sent through couriers on the above cited form address. Copy of Intra Bank Transection must be attached with application form. Forms are also available from account section of FPGMI, Lahore on Payment of Rs. 2000/-.
Bank Draft/Pay Order not accepted. The information given in the application form shall be verified at three (03) stages: – During the submission & processing of application. At the time of Acceptance/joining. During the training. If information submitted is found wrong, mismatched or forged, the application will be rejected and contract will be discontinued with no right of appeal at institution/govemment level. Applications received after due date or with incomplete information (Academic Info form) shall not be entertained.
