Attach Attested Photocopier of the Following Documents with UKJ Application:
• Matric Certificate. F.Sc Certificate or equivalent
• MBBS Degree with detail marks certificates of all professional
• Attempt Certificates
• PM&DC valid permanent registration Certificate
• House Job certificate with exact dates from the concerned Medical Superintendent as well as Professor/Head of the Department of concerned Hospital (Both are necessary)
• Domicile Certificate
• Computerized National Identity Card (CNIC)
• Attach experience certificate (if any).
• Three passport size photographs duly attested on the front/back (paste on the admission form)
• Those having foreign qualifications are required to submit the HEC/PMDC equivalency certificate
• Application forms are available from the account section of FPGMI. Lahore on Payment of Rs. 2000/-.
• Application with incomplete documents will not be considered for admission
• Admission is liable to be cancelled during the training if any discrepancy found in the submitted documents.
• Applications received after the due date or with incomplete information shall not be considered.
• Those applicants in service and desirous for admission on deputation basis must apply through proper channel
